- Performance evaluation forms and meetings must be completed and in Vision EAS by December 15.
- The revised DHRM Policy 1.40 Performance Management requires staff to complete a self-evaluation before the supervisor issues the performance evaluation form.
- Remember, there is new terminology for the rating scale.

Reminder for Step 2: Supervisors will only need to publish the performance evaluation form.
- Performance Evaluation- recommended to be published as a NEW document, but do NOT provide the employee with the option to "Use Editor Button" until you are ready to seek signatures.
Once you have published the performance evaluation form, it is recommended that supervisors schedule the evaluation meeting to ensure it can be completed by December 15.

- After editing the form in Vision EAS, the supervisor can review the approvers and lock the document for approvals. Watch this video for a demonstration on how to review the approvers and lock the document for approval. A transcript is also available. Three approvers are required for each evaluation: the direct supervisor, the reviewer (supervisor's supervisor), and the classified staff member.
- Once all approvers have approved, the document will go to the HR consultant to ensure it is complete prior to filing it in the electronic official personnel file.
