Changing your direct deposit information

To change your direct deposit information, please submit an HR service request by clicking on the Request Service button to ask for a Direct Deposit change form. Once we receive your request, a blank Direct Deposit change form will be sent to you.

Please complete the form and send it back to us along with bank account documentation such as a voided check or direct deposit instructions from your bank. The documentation should show your name, bank name, routing number, and account number.
