Open the SOSEC001 form at:

Uncheck all checked boxes. Check the VPN box under PC Applications. Print the form, and have your supervisor sign and date it. Scan the signed form and email it to tgaffney@vccs.edu. Please do NOT submit a ticket for ID requests. All ID requests must be sent through the Security office (via SOSEC001 form) per audited procedure.
